Job Description
A non-profit organization based in Philadelphia, PA that focuses in health disparities and provides support to individuals and families affected by HIV/AIDS, as well as other health and social disparities, is looking to add a Medical Case Manager to their team. This position will handle a desk of 40-50 clients and will involve on-site visits with clients.
Pay rate: $42,000-$45,000 (based on previous experience)
Location: Philadelphia PA (FULLY ON-SITE)
Requirements/Skills:
- Bachelor’s Degree in Social Work, Psychology, or any Human Services related field is required
- Experience with either HIV/AIDS and/or drug and alcohol related work
- Familiarity using Office 365
