Why does the job exist?

Provide persons, families, or vulnerable populations with the psychosocial support needed to cope with chronic, acute, or terminal illnesses, such as Alzheimer’s, cancer, or AIDS. Services include advising family care givers, providing patient education and counseling, and making necessary referrals for other social services.

How does it get done?

Employees in this Role assist in providing social work case practice services to clients with medical conditions, or in medical and public health settings and provide crisis intervention. Employees are assigned cases of minimal complexity and receive supervision in case decision-making. Employees provide community referrals, service referrals, communication with families and POA’s, schedule psychiatric/psychological counseling as needed, assist in active listening and counseling residents, participate with care plans, document progress notes and changes in conditions. Employee will also provide assistance in submitting PASRR; VA, Medicare and Medicaid benefits with the Administrative Service Department, and provides reporting to state and federal agencies as required.

Works cooperatively with all departments to ensure the comfort and needs of the resident are met. Are the resident’s personal advocate for health care within the facility. Employees evaluate cases independently; formulate clinical treatment plans; may provide direct psychotherapeutic services. Employees conduct interviews for intake, assessment, diagnostics, referral, reassessment, etc.; formulate interventions, placements, and service plans; determine initial and on-going eligibility/request for service; participate in conducting assessments and reassessments; coordinate and monitor service delivery; and provide crisis and/or emergency services.

Who are the customers?

The primary customers are the brave men and women that answered our nations call, executed their duties and now are unable to live in their respective communities due to failing health. This job will also support the widows of the service men and women to include parents that have lost children in combat (Gold Star Parents)

Ideal Candidate

Bachelor’s degree in Social Work (in accordance with NMAC 61-31-9) and one (1) years of experience in a social work setting such as long term care, social service agencies, healthcare, medical, educational settings, mental health, home health, hospice, developmental disabilities and other relevant experience, working with patients, clients, providers and families.

Minimum Qualification

Associate’s degree in Human Services or Social Sciences and two (2) years experience in mental health and/or substance abuse counseling. Substitutions Apply. See Substitution Table below.

Employment Requirements

This position is designated safety sensitive. Employment is subject to pre-employment and random testing, in accordance with all terms and conditions of federal and state law, rules, and regulations in relation to alcohol and/or drug testing.

Must possess and maintain a valid driver’s license.

Must possess and maintain a current Defensive Driving Course Certificate from the State of New Mexico or must pass and receive Defensive Driving Course Certification as a condition of continued employment.

Employment is subject to criminal background check pursuant to New Mexico Caregivers Criminal Screening Act, (Sections 29-17-2 through 29-17-5 NMSA 1978 and 7.1.9.1 through 7.1.9.11 NMAC) and is conditional pending results.

Successful completion of the Health Insurance Portability and Accountability Act (HIPAA) training.

Must sign and observe confidentiality agreement.

Must be able to read, write, speak and comprehend the English language.

Occasional travel for training & meetings and driving a State of NM owned motor vehicle maybe required; Working hours may include weekends, holidays, overtime, and callback hours. Essential functions of the job and specific physical ability requirements will be available at the time of interview or may be obtained at the New Mexico Veterans’ Home Human Resources office.

Working Conditions

Work is primarily in a office setting frequent work on computers, tablets, extensive writing, communication, telephone use, extended periods of sitting, walking, standing, listening, attendance at meetings. Frequent communicating and interacting with residents family, staff and the public; seeing close work such as typewritten or handwritten material; frequent exposure to VDT; frequent use of calculator & PC keyboard; must be able to function under pressure and under stressful situations. Occasional travel for training & meetings and driving a State of NM owned motor vehicle is required; occasional extended hours to meet deadlines. Working hours may include weekends, holidays, overtime and callback hours. Essential functions of the job and specific physical ability requirements will be available at the time of interview or may be obtained at the New Mexico Veterans’ Home Human Resources office.
