Schedule: Flexible between 8:30am-7:30pm Monday-Friday
Compensation: $44,000-$53,000/year salary

About the job:
A Mental Health Case Manager will provide coordination of services to support to deaf or hard of hearing children with mental illness and their families. Case Managers help coordinate services to support areas such as mental health, social, education, health, vocation, recreation, volunteer, advocacy, and transportation. Deaf and Hard of Hearing (DHH) Children’s Mental Health-Targeted Case Management Program (CMH-TCM) matches eligible children/families with a case manager who is culturally and linguistically competent to serve them and who is able to communicate directly with them using American Sign Language. The case manager has knowledge of culture that is specific to Deaf, Blind, and Hard of Hearing children and families.
Essentials:
- Provide services in the four core case management components-Assessment, Service plan Development, Referral and Linkage and Monitoring, and Coordination of Services
- Coordinate with a mental health professional to assure the completion of a mental health assessment of the child’s and family’s needs and to develop individual treatment plans.
- Develop and update the child’s individualized plan (IFCSP). Coordinate, monitor, and evaluate services that are outlined in the plan for appropriateness, progress, and continued service need.
- Coordinate services with the parent, child, and other members of the multi-disciplinary team.
- Develop and utilize available area resources and make appropriate referrals.
- Meet with the family and child as described in the treatment plan and program policies.
- Encourage and support parents in the parenting of their children.
- Keep necessary case notes documenting contact with program participant, family and collateral contacts.
- Maintain program participant case files.

Required Qualifications:
- Bachelor’s degree in Human Services or related field
- 2000 hours of supervised experience in the delivery of clinical services in the treatment of children with mental illness is preferred
- Fluency in American Sign Language
- Computer proficient
- Must have own vehicle and valid Driver’s License
- Proven skills: prioritizing, multi-tasking, organizing, interpersonal skills and networking
