Description
Provides field-based individual crisis intervention and supportive counseling with intensive case management. Provide evaluation; plan development; placement services; assistance in daily living; linkage and consultation; group counseling to homeless mentally ill individuals. JOB ID#35250A
POSITION RESPONSIBILITIES: Provide direct and indirect client services.
Participate in assertive assessment, evaluation and referral of homeless dual diagnosis clients. Assess client’s level psychosocial impairment, substance abuse history, self-care potential, family and support network availability, independent living needs, financial status, education, employment or training needs, and social/recreational needs. Refer those appropriate for intake at Cornerstone facility.
Complete all documentation within mandated time frame and in compliance with county, state, and Center guidelines. Provide crisis intervention – intervene with the client/others at the onset of a crisis to provide support and assistance in problem resolution and to coordinate or arrange for the provision of other needed services.
Complete all agency paperwork and client documentation on a daily basis or as required. Attend Cornerstone meetings as part of a multi-disciplinary team. Facilitate motivational groups; substance abuse, coping skills, conflict resolution.
Qualifications
The ideal candidate must a Bachelor’s degree in the field of Human Services, or related degrees with minimum of two years experience in counseling preferred.
Minimum of three years experience in behavioral health care with chronically mentally ill and homeless preferred. Familiar with DSM 5 preferred, Case management experience necessary.
Must act in accordance with all Health Insurance Portability and Accountability Act (HIPAA) of 1996 and related state law confidentiality requirements. Successfully complete the Center’s HIPAA training, pass the HIPAA test, and receive the Certification of Compliance.
Good driving record and must be insurable by Center’s insurance company is required. Bilingual, Spanish preferred.
