Description

Under supervision, to provide supportive counseling and case management services, within their scope of practice, to our clients within a psychosocial rehabilitative approach in order to facilitate their improved community functioning. To coordinate services in order to address clients issues of daily living either through individual or group modalities. Services include, but are not limited to, evaluation of client needs, collaboratively working with the client in developing goals/outcomes for treatment, advocating for and linking the client to needed resources and referrals, and providing crisis intervention services.

ESSENTIAL DUTIES AND RESPONSIBILITIES:

Provide supportive counseling and case management services, using both individual and group modalities in order to restore and/or maintain optimal functioning. To provide crisis intervention services in order to manage immediate concerns/risks, provide initial support/assistance, and help develop the working relationship between the client and clinic.

Manage a caseload of at least 100 clients.

Provide officer of the day coverage for crisis and walk-in clients as scheduled, and to screen clients for intake appointments and/or referrals.

ADDITIONAL DUTIES AND RESPONSIBILITIES:

Participate as a member of a multi-disciplinary treatment team. To attend all scheduled clinical debriefing, staff meetings, and team meetings in order to assist in assessing client’s needs, strengths, resources, and provide feedback regarding treatment planning.

Complete all chart documentation, reports, coordinated service plans, coordination plans, need assessments, correspondence within the time frame required, and within the guidelines set forth by the agency. The Department of Mental Health and MediCal/MediCare.

Be able to interface with the clinical and psychiatric staff in a professional and direct way.

Network effectively with community resources when providing linking and referral services for the clients.

Participate in home visits to clients when necessary and safe to do so.

Work at least one evening per week.

Other duties as assigned.

Qualifications

Bachelor’s degree in psychology, social work, or related field. Masters degree preferred.

Minimum of three (3) years professional experience working with the chronically mentally ill and in crisis intervention.

Ability to speak Spanish is desirable for some positions in the classification.

Knowledge of chronically mentally ill population, crisis intervention techniques, community resources, social security, and GR benefit packages.

May be required to have a current California Driver’s License and be insurable by the Center’s auto insurance company.
