Why does the job exist?

Provide direct medical social work services to clientele in Taos County (primary) and other NE Region counties as needed/ assigned. This position provides coordination for pediatric specialty outreach clinics and provides a safety net and assurance services both an individual/family level and on a systems level for children and youth with special healthcare needs.

How does it get done?

* Provide quality care coordination for Children & Youth with Special Health Needs who have complex medical, social and educational challenges by conducting assessments including financial/medical eligibility, psychosocial needs of the family, and transition from youth to adulthood.
*Coordinate support services, participate in staffing with schools, providers and other agencies identified as resources for the child/youth and families, while constantly advocating for the child in all general issues concerning the child’s well-being. Act as a liaison with other agencies and providers to insure coordinated care of the client.
o Assist clientele in cultural brokerage and language interpretation with medical systems and providers.
*Ensure that detailed documentation is complete and current, and medical records are reviewed and shared with proper entities in order to provide the highest quality services for the client.
*Assist with efforts to provides essential public health services during era of pandemic and other public health emergencies.

Children and Youth with Chronic medical conditions and their families.

Ideal Candidate

Bilingual (English/Spanish)
Social Work practice experience including experience conducting psychosocial assessments, intervention-service plans and referrals
Experience working in partnership with multiple agencies and/or in medical or public health setting.

Minimum Qualification

Bachelor’s degree in Social Work (in accordance with NMAC 61-31-9) and two (2) years of experience in a social work setting such as social service agencies, healthcare, medical, educational settings, mental health, home health, hospice, developmental disabilities and other relevant experience, working with patients, clients, providers and families. Employment Requirements

Must possess and maintain a valid driver’s license. Current license to practice as a social worker in New Mexico.

Working Conditions

Work is performed in an office setting, client homes and community settings. Some travel required. Work locations could be more than one county. Outreach to local/area in NE Region, Santa Fe, Rio Arriba, Los Alamos, San Miguel, Mora, Colfax, Taos, Union, Harding, and Guadalupe counties.
Clinic setting, constant public contact, computer usage, filing, involving stooping and reaching, may be required to lift up to 30 lbs.
