Summary

The primary role of the Nurse Delegated Training & Program Supervisor (ND TPS) is supervising all aspects of client care for their caseload. This role is responsible for coaching and developing new Nurse Delegated Coordinators.

Essential Duties and Responsibilities

  • Acknowledge and advocate for the needs, wants, and rights of clients with dignity and respect.
  • Identify areas of change that clients on their caseload would like to achieve.
  • Supervise individualized programs for clients and document activity thoroughly.
  • Supervise the administration, implementation, and documentation of medications, including nurse delegated tasks.
  • Accompany clients to health appointments and provide appropriate documentation for each appointment to management in a timely manner.
  • Completing daily, weekly, and monthly TPS checklists.
  • Monitor trainee’s schedules and communicate shift hours with Scheduling Manager.
  • Assist staff with finding appropriate times to conduct peer coaching.
  • Provide peer coaching and development for new staff, complete documentation.
  • Perform coaching meetings with new staff in training utilizing SMART goals and Reflection sheets.
  • Conduct 1:1s with all staff weekly.
  • Provide training, assistance, and support to staff in all areas of daily living as outlined in the client’s care plans.
  • Maintain open communication with supervisor and management team to report any concerns with clients, scheduling, and/or other staff.
  • Maintain appropriate staff/client relationships.
  • Coordinate client outings and activities.
  • Coordinate transportation for clients to and from all agency activities; arrange transportation services as appropriate.
  • Assist with client money management such as petty cash, receipts, checkbook and EBT’s.
  • Supervise homes to ensure adequate client and staff safety.
  • Delegate appropriate tasks to staff with guidance and verbal training before sending them out on appointment or task.
  • Report promptly instances of injuries, behaviors, abuse, and safety hazards, etc. to management, following the Chain of Command.
  • Ensure adequate health care for each client.
  • Monitor and re-order medications that are not given on a regular base or medications that are not ordered in client’s regular medication cycle (PRN’s).
  • Assist clients with mobility needs, transferring them from wheelchair to bed.
  • Monitoring vitals (if applicable) and client behavior.
  • Turning or adjusting clients in bed to prevent bedsores and/or other discomfort.
  • Ensure clients have access to regular meals.
  • Communicate with medical offices, pharmacies, and the nurse delegator.
  • Administer medications to clients and document accordingly, including nurse delegated tasks.
  • Assist client with daily hygiene as outlined in care plans, including personal care such as brushing teeth, bathing, hair washing, toileting, etc.
  • Maintain client confidentially.
  • Maintain an ongoing inventory of client’s personal property.
  • Safeguard all company and client property.
  • Abstain from partaking of or possessing any illegal drugs or alcohol while on duty.
  • Be aware and abide by all agency policies, procedures, and practices.
  • Perform other responsibilities as defined by management.

Education and Experience

  • Minimum age of 18 years
  • Current criminal background check cleared by DSHS before working alone unsupervised with the clients
  • Valid driver’s license or Washington State ID
  • Home telephone or cell phone
  • High school graduate or equivalent
  • Additional training specific to client complex care and behavioral needs
  • Ability to follow instructions, organize and set priorities, work well with other staff and clients, exhibit job-related competency, and make independent judgments.
  • Ability to supervise new hires.
  • Preferred to have a minimum of one year experience working with developmentally disabled adults.

Certificates, Licenses and Registrations

  • Nursing Assistant Registered (NAR) License, or higher (required)
  • Home Care Aide (HCA) License (preferred)
  • Certified Nursing Assistant (NAC) License (preferred)
  • The Buzz on Adult Leaners Certificate (preferred)
  • Peer Coaching Training Certificate (preferred)
  • Current CPR / First Aid Certificate (preferred)
  • Current Bloodborne Pathogens Certificate (preferred)
  • Nursing Assistant Registered License, Home Care Aide License, or Certified Nursing Assistant License (preferred)

Knowledge, Skills and Abilities

Language Skills: Excellent reading comprehension and written communication skills required. For example: ability to read, analyze, and interpret documents, reports, technical specifications, governmental regulations, and correspondence. Ability to apply proper grammar, spelling, punctuation, and format to communications. Ability to present information in one-on-one, small group, and large group situations to management and staff.

Mathematical Skills: Ability to work with mathematical concepts such as probability and statistical inference; ability to compute discount, interest, profit, loss, ratios and percentages; and ability to draw and interpret graphs.

Reasoning Ability: Ability to solve a wide range of practical problems and deal with a variety of concrete and complex variables; ability to interpret a variety of instructions furnished in written, oral, diagram, or schedule form; and ability to deal with multiple variables.

Computer Skills: Ability to operate related computer applications (i.e. word processing and spreadsheet applications), and business equipment including personal computer, copy machine, fax machine, and telephone.

Physical Demands

Talking: Able to convey detailed or important instructions or ideas accurately, loudly, or quickly.

Average Hearing: Able to hear average or normal conversations and receive ordinary information.

Finger Dexterity: Using primarily just the fingers to make small movements such as typing, picking up small objects, or pinching fingers together.

Repetitive Motion: Movements frequently and regularly required using the wrists, hands, and/or fingers.

Average Visual Abilities: Average, ordinary visual acuity necessary to prepare or inspect documents or products, or operate machinery.

Physical Strength: Frequently required to sit, stand, and walk and occasionally required to stoop, kneel, crouch, or crawl. Occasionally required to lift and/or move up to 20 lbs.

Your Work Place

Externally, work is normally performed in a home setting, where exposure to conditions of extreme heat/cold, poor ventilation, and fumes are very limited. Noise level is moderate and may include sounds of normal house hold equipment (furnaces, telephones, etc.). No known environmental hazards are encountered in the normal performance of job duties.
