Nurse Supervisor for Assisted Living, Brooklyn, NY

Job Description

We are looking for a Nurse Supervisor to work for one of our clients, a multi-service certified home care organization, located in Brooklyn, NY.

This is a permanent, full-time job.

In this role you will be responsible for overall management and oversight of Adult Care Facilities and ensure the provision of quality patient care and appropriate provision of services.

An ideal candidate would be a graduate of accredited school of nursing, and currently registered in New York State as a Registered Professional Nurse.

Must have a mandatory 2 years’ experience with a licensed home care agency.

Knowledge of State and Federal regulations as it pertains to Certified Home Health Agency required.

Responsibilities include, but not limited to:

  • Responsible for staffing and scheduling in the Adult Care Facilities department to ensure appropriate caseloads taking into consideration, volume, acuity levels and intensity of treatment plan.
  • Participates in the on-call rotation to ensure 24 hour agency coverage.
  • Reviews On-Call Report on a daily basis to identify issues requiring follow-up.
  • Provides ongoing support to nurses in the ACF regarding problem solving, assistance with implementation of the plan of care and management of complex- care patients.
  • Assists in staff development with providing ongoing education related to regulatory updates, new/revised agency policy and procedure, professional updates, workshops, orientation of new staff and programs related to clinical practice.
  • Evaluates job performance on an ongoing basis through case conferencing, review of documentation and on-site supervisory visits.
  • Reviews and acts upon all applicable reports such as: outstanding 485 report, outstanding documentation report (OASIS, revisits), interim orders, in order to ensure regulatory compliance, adherence to agency’s policies and procedures, monitor workflow, identify problem areas requiring correction and to manage overall daily operations. Resolves outstanding issues in a timely manner.
  • Investigates complaints registered by MD, patients, family members, other ACF personnel. Submits findings, outcomes and resolution to quality assurance department.
  • Interviews of potential candidates and makes recommendations for hiring.
  • Participates in Performance Improvement activities as assigned.
  • All other duties as assigned.
