Position Summary:

The LifeMoves Outreach teams are seeking a self-driven and client-centered Outreach Programs Associate to efficiently support the fast-paced rapid response and referral processes for the multiple outreach programs that currently exist throughout San Mateo County. The successful candidate will exhibit exceptional communication and interpersonal skills, as well as the ability to represent LifeMoves Outreach in a diplomatic and professional manner amongst community partners, staff, clients, funders, and various city/county entities.

Primary Duties:

  • Serve as the initial point of contact for community stakeholders seeking outreach responses.
  • Promptly answer outreach hotline, screening incoming referrals by assembling information regarding potential participant’s background and needs.
  • Delegate referrals to various outreach agencies and coordinate appropriate follow up efforts.
  • Establish and maintain strong professional relationships with community partners and referring personnel.
  • Effectively resolve broader concerns from referring entities and notify leadership of critical issues.
  • Maintain program and participant records, including referrals tracking, in accordance with applicable standards and regulations, grant requirements, etc.
  • Assist Program Director in compiling and evaluating program metrics, policies, and procedures.
  • Participate in continuing education activities, remaining knowledgeable in areas of customer service, homelessness, and existing community barriers.
  • Maintain a high level of ethical conduct regarding confidentiality, dual-relationships, and professional stature.
  • Attend regular meetings and trainings as assigned by Program Director.
  • Quickly evolving position; additional duties subject to change at any time per supervisor request.

Position Qualifications

  • High School Diploma or GED required.
  • Minimum of two years professional experience in customer-service related field required.
  • Computer literacy and proficiency in Microsoft Office and Excel required.
  • Automobile, valid driver’s license, and clean driving record required.
  • Bachelor’s degree in social services or related fields preferred.
  • Professional or personal experience working with the unsheltered population, those at risk of homelessness, and/or other marginalized populations struggling against systemic inequalities preferred.
  • Professional or personal experience working with individuals who could benefit from behavioral health resources and/or working with dually diagnosed individuals preferred.
  • Bilingual (Spanish) capabilities preferred.
  • Availability to work occasional evenings and weekends preferred.
  • An understanding of, or willingness to learn about, ideas and practices that promote diversity, equity and inclusion, and a commitment to ongoing professional development in this area.
  • Strong work ethic; comfortable working independently; keen attention to detail and highly organized.
  • Excellent communication and interpersonal skills; proven ability to collaborate with other entities and resolve conflict.

Physical Activity Requirements:

  • Ability to lift/support at least 30 lbs. as needed
  • Ability to sit, stand, stoop and bend frequently
  • Ability to sit and keyboard as needed
