Job Description:
The Department of Neurosurgery is seeking a Clinical Programs Coordinator III to serve in the role of patient discharge navigator. The Patient Discharge Navigator is a highly responsible position that will work closely with the department’s physicians, physician extenders and Clinical Service Representatives to serve as a liaison between the patient and patient care team to assist in mitigating post-op (post-operation) complications. This position will also assist with triaging post-op patient calls and assist the care team with other patient care coordination activities.
Conduct routine telephone screening calls to post-discharge patients.
- Conduct post-discharge screening phone calls with all neurosurgery patients discharged from the hospital or UF medical facility. The post-discharge screening process will follow the Department’s post-discharge protocol and references to the post-discharge instructions specific for the patient.
- Assure that any medical issues, clinical questions, or complications that occur during a call are immediately routed back to the provider, nurse practitioner or physician assistant.
Patient Care Coordination
- Assist with scheduling follow-up appointments, image studies, complete necessary authorizations for Physical Therapy or Occupational Therapy.
- Work with clinical service representatives if patients require to schedule surgeries or procedures.
Triage/Routing Inbound Calls
- Based on assigned duties, the Patient Discharge Navigator can be assigned to answer patient calls for the Department. The Navigator will document message, triage and route information directly to the care team to source a solution. This may include, but not limited to: assuring patients receive a call back for any medical questions, medications changes, refills, or questions regarding their medication plan, FMLA paperwork, Return to Work Authorizations.
Expected Salary:
$76,120-$84,000 commensurate with education and experience
Minimum Requirements:
Bachelor’s degree in an appropriate area and four years of relevant experience; or an equivalent combination of education and experience.
Preferred Qualifications:
• Experience within a neurosurgical practice and/or surgical patient care experience
• EPIC Experience and/or experience working in electronic health records
• Demonstrated ability to provide exceptional Customer Service Skills
• Ability to demonstrate compassion and empathy with patients
• Demonstrated ability to Multi-task
• Demonstrated ability to be flexible within job parameters.
• Medical Assistant Certification
• Superior communication and patient/customer-service skills, and ability to demonstrate compassion and empathy with patients.
• Knowledge of that includes the knowledge of CPT and ICD-10 coding guidelines as well as basic medical terminology.
