The mission of the Colorado Coalition for the Homeless is to work collaboratively toward the prevention of homelessness and the creation of lasting solutions for homeless and at-risk families, children, and individuals throughout Colorado. The Coalition advocates for and provides a continuum of housing and a variety of services to improve the health, well-being, and stability of those it serves.
The Colorado Coalition for the Homeless seeks an Case Manager I, Beacon Place to provide support, connect to resources and benefits, and implement activities in the CCH transitional housing facility. The ultimate goal is to assist these individuals in maintaining their housing, move on to a more permanent housing setting, and provide the services and linkages necessary to prevent them from returning to homelessness. This position is located at 3636 W. Colfax Ave., Denver, Colorado 80204 and reports to the Program Manager.
NOTE: In order to ensure that our consumers receive the best possible care the candidate chosen to fill this position will be required to complete our internal credentialing and privileging process prior to a start date. The credentialing and privileging timeline is dependent on the selected candidate’s submission of documents necessary for clearance.Please note that a start date will be scheduled once credentialing and privileging is complete.

Essential Job Functions
- Provide excellent internal and external customer service;
- Assist resident in the development of individualized goal and service plans and working toward the goals in their service plan;
- Ensure that each resident receives the support and services they require to maintain their housing and move on to permanent housing;
- Support residents in attaining the most appropriate level of agency & independence;
- Collaborate with colleagues, clients, and management staff in problem solving resident and facility challenges, including those that may jeopardize resident’s housing stability;
- Create and implement social opportunities for residents;
- Facilitate group opportunities for independent living skills for residents;
- Coordinate and/or provide transportation to medical, behavioral health, housing, financial, and other supportive activities while encouraging independence when possible;
- Assist residents with obtaining and maintaining benefits;
- Drive facility van as required;
- Ensure all verification paperwork is completed as required by law and/or payor sources;
- Provide orientation to all new residents;
- Assess strengths and needs of all new residents;
- Develop interventions or linkages to address resident needs;
- Provide hands on assistance and/or training to residents in order to assist them with ADLs (Adult Daily Living Skills) and in maintaining their housing is a safe and sanitary manner;
- Provide crisis intervention as required;
- Update all paper and computer work, including resident files and goal plans, as required;
- Serve as liaison between resident and other service providers;
- Collaborate and practice in a team environment to ensure that the facility operates optimally through covering other duties, including light housekeeping, front desk coverage (at least 4 hours per week), assisting residents in cleaning, answering phones, etc.;
- Maintains current and complete COVID-19 vaccination(s), as defined by the Centers for Disease Control and Prevention and local health authorities.

Qualifications Summary
- Bachelor’s degree in a related field or equivalent experience in similar program(s).
- Previous case management experience preferred.
- Experience in residential setting preferred.
