The role of the PCA Team Leader is coordinate and assist with the provision of Nursing Care to Residents and to oversee the delivery of care by assigned Personal Care Assistants in accordance to the policies and procedures along with the Mission and Core Values of Fellowship Community and DHS regulations.
ESSENTIAL QUALIFICATIONS / CREDENTIALS
Must have knowledge of DHS regulations related to Personal Care.
Must have certificate in Basic Life Support.
Must have computer skills at an intermediate level.
Ability to read, write, speak and comprehend English.
Must have a High School Diploma or equivalent.
Knowledge of the special needs and care requirements of geriatric residents
Knowledge of medical terminology
Able to effectively present information in one-on-one and small group situations to customers and other employees of the organization.
Works well with others in a positive and cooperative manner
Personal Care Home Medication Training, CPR and First Aide qualified or willing to pursue

Meets the requirements of the Pre-Hire Assessment.
Essential Duties and responsibilities of resident care
§ Attends in-service training as required.
§ Performs all tasks completely as outlined in the PCA assignment book.
§ Must give complete and accurate report to PCA Team Leader on oncoming shift to allow for safe continuity of care.
§ Works in conjunction with PCA staff to perform good resident care.
§ Assists with admissions process as directed by the Director of Resident Care.
§ Assists with activities as needed.
§ Supports the Food Service Staff.
§ Attends Family Conferences as requested.
§ Monitors PCA performance to ensure that the proper techniques and procedures are being followed. Reports any unsafe concerns to the Nursing Supervisor in a timely manner.
§ Administers medications to residents in accordance with the DHS regulations and the policies and procedures of Fellowship Community.
§ Administers insulin, as well as performs respiratory therapy (nebulizers, oxygen) per the Diabetic and Respiratory Required Education.
§ Completes monthly re-caps and/or Quarterly Resident Assessments per the direction of the Director of Resident Care and/or the Nursing Supervisor.
§ Completes duties as directed on Weekly Staffing Audits.
§ Follows established infection control and OSHA guidelines in accordance with facility’s policies and procedures.
§ Ensures accurate, timely documentation of records. Reviews Tx book to ensure PCA is also doing their initial completion each shift.
§ Thins charts (nightshift) to appropriate content as directed.
§ Ensures that each resident is provided privacy and knocks before entering resident’s room.
§ Treats all residents fairly and with kindness, dignity and respect.
§ Creates and maintains an atmosphere of warmth, personal interest and positive emphasis.
§ Oversees the work of the PCA’s and Team Leaders on their shift and monitors such to ensure that the proper techniques and procedures are being followed.
