Description
Position Overview
This case manager will provide one-on-one supportive services to youth preparing to join the workforce. The Case Manager will assess basic needs, identify barriers to employment, develop an Individual Employment Plan to address those barriers, and coordinate services to help youth prepare for and secure living wage jobs. This position is located at Northwest Plaza, St. Louis MO.
Essential/Primary Functions
1. Understands and applies MERS policies and conducts business in a manner that represents professional standards of excellence.
2. Assists customers with developing and modifying individualized education and career plans with input from referring deputy juvenile officers.
3. Meets with customers, MERS staff members, referring deputy juvenile officers, and other stakeholders on a regular basis to discuss progress and plan to more effectively serve customers to achieve desired outcomes.
4. Prepares written reports at predetermined service intervals and at the conclusion of services and distributes them to customers, referring counselors, and other stakeholders, as appropriate.
5. Assists mental health coordinator and job developer with providing direct services including but not limited to obtaining interview clothes, transportation to doctors’ appointments, job retention, and community support services.
6. Maintains strict adherence to standards of professional conduct regarding confidentiality of customer information.
7. Maintains accurate, complete, and up-to-date records for all consumers on caseload.
8 Develops, strengthens, and expands professional relationships with other community resources on behalf of current and potential customers.
9. Contributes to the financial success and quality control of the MERS/Goodwill.

Qualifications
Knowledge and Abilities:
1. Ability to motivate and communicate with consumers and staff
2. Ability to coordinate with and relate to business professionals, trainees, and agency staff
3. Computer literacy
4. Excellent written and verbal skills
Education and Experience:
1. Bachelor’s degree in Psychology, Vocational Counseling, Education, or a related field is required
2. Minimum of one year experience working with at risk youth.
3. 2-4 years of case management experience.
