Description
Position Summary:
Includes the following; responsibilities for insuring that clients receive all appropriate services, as well as performance of other work related duties as assigned:
Essential Job Functions:
- Responsible for insuring program participants and clients receive all appropriate services
- Provide care coordination services provided by ancillary agencies, HIV treatment, primary health care, housing, employment training employment, public entitlements, etc.
- Conducting community-based outreach activities
- Recruit and assess clients needs to establish service plan.
- Identify and evaluate all areas of support that the clients needs, physical, medical, social, psychological, environmental and economic resources.
- Implement, monitor and track the service plan.
- Prepare case management interview, file reports of all kinds
- Participates in agency Case Management activities and completes case management tasks as indicated
- Create and Coordinate educational fairs and outreach events
- Offer educational prevention talks
- Plan and implement development activities in life skills.
- Document interventions in the service file.
- Maintains accurate documentation of client case management activities
- Coordinates the location of participants if necessary.
- Providing brief counseling services
- Help with the Coordination and works at educational fairs and outreach events.
- Coordinate efforts with service providers in the community
- Attend and participated in training activities, staff meetings, and supervisory sessions
- Perform every task to get the job done.
- Fulfill all functions for purposes of your post and those delegated by the Lead Navigator.
- Other duties as assigned by the Program Coordinator.
Qualifications
Essential Qualifications:
Education:
Bachelors Degree in Social Work and at least 2 years experience working in prevention programs, HIV and substance abuse in youth. Capacity to learn and implement intervention clinical core elements. Case management experience preferred and strong computer skills. Knowledge of the evidence based practices
Experience and Competencies:
Two years working with the youth 13 24 year of age population, community organizations and the business community. Strong organizational skills and follow-up skills, excellent written and verbal communication skills. Proficient in Microsoft Office programs. Ability to present program information to community stakeholders.
Have taken courses or training in the prevention of transmission and sexual diseases and the problematic use of controlled substances.
