Description

Position Summary:

The person in this position will be responsible for oversight of the daily administrative operation of HART TCE as well as the planning and coordination of clinical activities pertaining to services for consumers enrolled in HART program. The Program Coordinator oversees the admissions for consumers referred from outside agencies, assessments for consumers and provides individual and family therapy, as needed. The Program coordinator is responsible for keeping the Program Director of informed of progress of all services delivered to the consumers internally and through external referral sources.

Essential Job Functions:

  • Assists in the daily operations of the program and provides specialized leadership under the supervision of the Program Director
  • Identify target populations and locations where service gaps exist.
  • Coordinating HART TCE participation at health fairs and other community events.
  • Provide community awareness, identify, and recruit consumers that meets program eligibility requirements by attending health fairs and other community events, as needed
  • Represent the agency at meetings with funding agencies, governmental authorities, seminars, workshops, community partners, etc.
  • Maintains relationships with referring and supporting agencies
  • Responsible for the implementation, maintenance, and coordination of the social media campaign and marketing efforts for recruitment and retention.
  • Prepare reports on monthly, quarterly and yearly basis, or whenever requested to do so by Program Director.
  • Monitors the systems, procedures, and standards of the program, informing the appropriate party when deviations occur.
  • Administer verbal reprimands and learning experiences to staff according to agency’s policies and procedures
  • Ensures fidelity of evidence based practice models with quarterly chart reviews.
  • Arranges for staff development, and training in coordination with Program Director.
  • Ensures enrollment and completion rates remain in compliance with grant requirements
  • Receives, schedules and monitors the intake process of all referrals from outside agencies, assigning to primary therapist as appropriate.
  • Conducts Case Conference weekly with HART TCE program staff.
  • Monitors and assures accuracy, timeliness and consistency of data, SISAR, discharge summaries and wellness & recovery plans, making comments and notes on to be discussed in weekly Case Conference.
  • Provides direct clinical and treatment supervision in coordination with Clinical Coordinator
  • Oversees discharge/transfer process, to ensure appropriateness and continuity of care for transfers within and discharges from the program, consulting with Program Director and Clinical Coordinator, as needed
  • Conduct admission and Biopsychosocial assessments using the SMQ-10, as needed.
  • Prepare and review Interpretive Summaries to ensure they are based on information obtained from the assessment, as needed.
  • Develop Wellness and Recovery Plans in collaboration with the consumer, as needed.
  • Conduct monthly Wellness and Recovery Plan Reviews with the consumer, as needed.
  • Provide in-home/on-site individual therapy, as needed
  • Provide in-home/on-site family therapy, as needed.
  • Provide CLEAR risk reduction counseling, as needed
  • Document all clinical services and activities in accord with licensing, Medicaid, and Joint Commission standards and agency policy and procedure
  • Provide referral and linkage for ancillary services for the consumer, as needed
  • Write monthly Progress Reports regarding assigned consumers for courts, probation departments, Department of Corrections, and employers as needed.
  • Prepare Discharge Summaries for assigned cases, as needed

Qualifications

Essential Qualifications:

Certifications/Licensures:

  • Must hold a current professional license in the state of Florida (LCSW, LMFT, LMHC, or similar-like)

Education:

  • Graduation from an accredited masters program with a degree in Addiction Counseling, Criminal Justice, Social Work, Psychology or related field is required

Experience and Competencies:

  • Two years of previously supervised work experience in providing individual/family therapy is preferred
  • Bilingual is preferred.
  • CPR Certification, First Aid Certification and an annual Tuberculosis test.
  • Ability utilize Microsoft Office Software Suite
  • Ability to provide an Affidavit of Good Moral Character
  • Clearance in Pre-Employment Drug Testing and Criminal Records Checks, as required.
