Description

Coordinate day to day operations of the Street Based Outreach team funded by LAHSA in collaboration with CD3. Coordinate outreach efforts with council office, LAHSA and other community partners. Supervise and support program staff in adhering to Center policy and job responsibilities. Facilitate all Street Based team consultations, team dispositions, staff meeting or other meetings as Directed by Director and/ or Assistant Director. Assist in supervision and annual Performance Evaluations of multi-disciplinary treatment team. Provide clinically appropriate treatment responses to staff and clients in order to maintain stability and integrity of the program. Attend all Center Administrative meetings and provide back-up to Director/ Assistant Director in LAHSA/ county meetings or those meetings as assigned by Director/Assistant Director and/or Director. Attend Center’s Quality Assurance and Utilization Review meetings as requested.

POSITION RESPONSIBILITIES:

Will supervise assessments and evaluation of seriously and persistently mentally ill adults. Determine individual’s strengths, needs, and resources. Assess client’s level of psycho-social impairment, self-care potential, family and support network availability, independent living needs, financial status, educational, employment or training needs, and social-recreational needs.

Oversee day to day functioning of the Street Outreach team, including productivity, caseloads, service provision, and timeliness of all required paperwork.

Oversee and delegate outreach/ engagement coordination within the SPA 2 team/agencies and provide resources to facilitate linkage to treatment, housing and resources.

Conduct case disposition meetings for the Street Outreach team to review each participants engagement, HMIS, CES Packet and other enrollment documentation. Assess potential crisis needs of clients and develop and implement intervention plans to de-escalate crises. .

Monitor HMIS data entry and reports as needed per CD3 and/or Director/ Assistant Director.

Provide on-going daily supervision and support to staff in assessing adequacy and appropriateness of clients living or funding arrangements and assist in securing alternative living or funding arrangements as needed.

Provide crisis intervention as needed and intervene with clients/others at onset of crisis to coordinate and arrange for provision of needed services.

Qualifications

EDUCATION:

The ideal candidate must be Licensed Mental Health Professional (LCSW, MFT, PSY.D. or Ph.D.)

EXPERIENCE:

Minimum of two years’ experience in mental health field working with experience with homeless chronic mentally ill. Knowledge of DSM V required. License in current standing with the BBSE, Bi-lingual/Bi-cultural a plus.

Knowledge of related community resources for treatment of substance abuse, mental illness, and domestic violence.

Ability to communicate and work with a culturally diverse adult population as part of a multi-disciplinary assessment, referral and case management team.

Must have a current California driver’s license and be insurable by the Center’s auto insurance carrier.

Ability to speak Spanish is desirable.
