Purpose: (General description and summary)

The Program Manager, Supported Housing Peer Services [Program Manager] position is responsible for oversight of the Breitung Building Resident Services programming, supervision of staff, assurance of quality service and collaborative relationships with all stakeholders, including property management, building owner, State, County, and community agencies.

The Breitung Building is comprised of 28 Housing and Urban Development – Veterans Affairs Supportive Housing [HUD-VASH] permanent supportive housing units. Residents include veterans who have experienced houselessness, mental health, and/or substance use challenges to support them in obtaining and maintaining permanent supportive housing, using a variety of approaches that support the whole person.

The Program Manager is responsible for administrative supervision and consultation to Bridging Connection and Cultivating Community (BC/CC) Peer Specialists. Position collaborates with on-site program management to support oversight of direct reports. Both programs providing transitional housing and wraparound supports in a motel environment, designed to support mental wellness, stable housing, and transition to permanent housing, while utilizing a variety of approaches that supports the whole person.

The Program Manager will work with a variety of stakeholders including Horizon Property Management, the Joint Office of Homeless Services, Veterans Affairs, Multnomah County Behavioral Health, and the New Narrative Leadership Team to ensure implementation of best practices, positive program outcomes, and continued quality service while upholding the New Narrative Mission and Values.

Accountabilities: (Responsibilities of the job)

Peer Program Leadership Activities:

  • Leads the provision of Resident Services from a peer support perspective, providing culturally responsive, trauma-informed services and incorporating best practice techniques into the program.
  • Exudes and models New Narrative’s values and mission in all interactions.
  • Leads programming in alignment with the Peer Services Vision and Strategy.
  • Upholds the National Association of Peer Supporters Guidelines for Peer Specialists and Supervisors, Intentional Peer Support, and the 12 Core Competencies of the Substance Abuse and Mental Health Services Administration (SAMHSA) throughout programming.
  • Conducts scheduling of staff; ensures 24/7 staffing, reviews, assess, and approves Time off requests, while evaluating the availability of staff.
  • Works to fill gaps in the schedule and absences in a timely manner.
  • Provides minimum bi-weekly program supervision to supervisees. The position is responsible for professional development and performance management such as annual reviews.
  • Conducts group supervision in program team meetings and provides training as needed.
  • Provides day to day support to staff for questions, consultation, and other collaborative and supportive efforts; includes but not limited to, coverage for absent staff ensuring consistent participant support.
  • This position will have dual focus, splitting attention, and commitment to be approximately 70% overseeing the Breitung program and 30% BC/CC to providing on-site support to peer specialists.
  • Maintains collaboration and communication across team and stakeholders; supports cohesive and seamless resident experience.
  • Manages referrals, checks accuracy of enrollments in the database and oversees the program and eligibility requirements.
  • Ensures all documentation is completed within 24-48 hours in electronic health record.
  • Maintains program budget, technology tracking, and reporting requirements for grant and/or contract funding as well as Memorandum of Understanding [MOU] with partners in collaboration with Director of Peer Support Services.
  • Oversees personal and staff expense reports, ensures expense reports are submitted weekly and completed monthly.
  • Approve staff timecards within the deadline provided.
  • Builds and maintains effective community partner relationships, including referral partner relationships, to support mutual access and awareness of area services and what Breitung offers
  • Attends all required meetings with county and external community partners in collaboration with Director of Peer Support Services
  • Works with Quality Assurance Manager to conduct case review and OAR compliance.
  • Reviews documentation, IRs, and performance, and provide clear and direct feedback to support best practice with person centered, trauma informed, harm reduction, inclusive, lens.
  • Proactively, sincerely, and regularly assess and evaluates resident services by soliciting feedback from residents and staff to continuously improve policies and procedures. Adjust programming as needed.
  • Provides feedback to staff; addresses issues of quality care and/or utilization of services through an organized performance improvement process.
  • Partners with other staff members to address and resolve stakeholder issues and complaints.
  • Maintain a regular, reliable work schedule.
  • Is on-call for after-hours consultation and crisis response as needed.
  • Serves as liaison for Multnomah County staff, coordinating site reviews, audits, chart reviews, and certificate requirements. Attends stakeholder meetings as required.

Peer Support with Residents:

  • May provide direct support to resident as well as coaching and oversight to staff providing same.
  • Works with residents to identify wellness and recovery goals.
  • Assists in creating a Wellness Plan that identifies the individual’s self-determined goals, as well as specific objectives to carry out these goals.
  • Exemplifies recovery principles in order to show by example that recovery is possible and attainable. Shares own recovery story as appropriate and walks the road of recovery alongside the resident.
  • Assists with screenings, move-in, move-out and activities necessary to maintain residence as requested by residents.
  • Works with teams, including clinical and property management team members, to provide an integrative experience for resident that supports them holistically in goal-setting and daily living.
  • Develops and facilitates peer-led groups, with input from community members.
  • Coordinates and consults with other team members to keep team informed of issues and progress.
  • Ensures timely completion of all required paperwork, including documentation of all resident centered interactions
  • Works to increase level of cultural sensitivity, awareness and competency regarding Person Centered and Peer Delivered Services.
  • Educates and advocates for the values and principles of the Recovery Model.

Crisis Prevention/De-escalation/Harm Reduction

  • Provides supportive presence through regularly walking the community to ensure entrances are secure, communal areas are clean and free from clutter, address resident issues and concerns.
  • Supports residents in maintaining lease requirements and remedying any violations
  • Provides, supports, and supervises crisis response and de-escalation in a community- based setting.
  • Utilizes best practice crisis prevention approaches to decrease use of crisis services, Project Respond, EMS, police, hospitalization, etc.
  • Responds to emergency situations and work in collaboration with law enforcement, emergency health care providers, and acute care settings if necessary.
  • Supports harm reduction principles.
  • Linkage to resources such as, but not limited to: clean needle exchange, NARCAN, MAT, SUD services, education around harm reduction, etc.
  • Adheres to policy and procedure for mandatory reporting and documentation.

Qualifications:

  • Peer Support Certification required(PSS, PWS, or CRM)
  • 1 year of direct support experience in the mental wellness field required, 1 year of direct support experience as a peer specialist preferred.
  • Minimum of one-year supervisory experience required.
  • Lived experience with either mental health, substance use, houselessness, restrictive environments, and/or altered states required.
  • Lived experience with military service/veteran status preferred.
  • Bilingual (Spanish/English) preferred.
  • Incumbents with a history of substance use challenges must be in recovery and free of problems relating to active substance use for 1 full year preceding date of hire and must maintain this recovery.
  • Must maintain current Oregon or Washington State’s driver’s license as position requires the ability to drive.
