Description
The Population Health Advisor (PHA) will participate in all aspects of the agency’s Sexually Transmitted Infection (STI) Prevention and Treatment Program. Responsibilities include providing STI risk reduction education, as well as providing positive STI-related lab results and scheduling follow-up appointments for treatment based on the Medical Provider’s determined plan of care. The PHA will work in conjunction with Registered Nurses to provide additional support to patients with STI-related health education and scheduling follow-up care. Additional responsibilities include the coordination and timely reporting to the NYCDOHMH of reportable communicable diseases identified at Callen-Lorde.
SPECIFIC RESPONSIBILITIES/ESSENTIAL FUNCTIONS
- In collaboration with Medical Providers and Triage Nurses, notifies patients of positive STI results, relays treatment information and partner notification standards to patients, and schedules follow-up visits, including visits for treatment.
- Maintains required documentation for all activities in appropriate record-keeping systems as per documentation guidelines.
- Coordinates and assures timely reporting to the NYCDOHMH of reportable communicable diseases identified at Callen-Lorde including data entry of cases and reports.
- Follows all internal protocols for STI Prevention Program services and works toward meeting the targets of STI prevention contracts and clinical quality indicators.
- Builds relationships and ensures collaboration between departments and outside agencies.
- Represents Callen-Lorde at and/or attends professional conferences, in-service trainings, and meetings at the request of or with the approval of the supervisor.
- Participate and be a champion of Callen-Lorde’s diversity, equity, and inclusion initiatives.
- Performs other duties, as assigned by supervisors, to maintain the efficiency and effectiveness of STI-related services.
Qualifications
Education, Experience
- College degree (BA/BS) in health/social services required (relevant work experience considered).
- Experience in health care, HIV/STI service delivery, or social services preferred.
- Demonstrated knowledge of HIV/AIDS, Sexually Transmitted Infections (STIs), and transmission risks.
- Demonstrated knowledge of risk reduction theory as it applies to transmission of HIV and other STIs.
- Familiarity with STI and/or communicable disease reporting.
- Experience with data management and data management systems.
