Preferred Qualifications: Familiarity with family health and/or chronic disease activities and prevention and control activities related to cancer, diabetes, cardiovascular disease, and/or asthma. Experience in oversight and evaluation of public health and preventive health programs, as well as promoting public health messages. Experience managing multiple tasks, developing and delivering educational materials, coordinating training activities, and contributing on policy development. Excellent organizational, interpersonal, and written and oral communication skills. Ability to work both independently and as a member of a team. Proficiency with computer-based software packages such as Microsoft Office, Outlook, etc
Duties Description The Public Health Program Nurse (PHPN) will work in collaboration with the Central NY Environmental Health Lead Field Coordinator to provide training to local Health Departments staff regarding Lead Program guidelines and expectations; to provide technical assistance on cases and workplans; and to collaborate with the Regional Lead Resource Center. The incumbent will work to improve the health of communities in need by providing training, technical assistance, resources, guidance, and clinical direction on cancer prevention/screening/survivorship/peer education, diabetes activities, implementation of the Prevention Agenda and implementation of the Chronic Disease Action Plan to local health departments, community-based organizations, and other community partners. The PHPN will serve as a resource to CNYRO maternal and child health programs. The PHPN will facilitate enhanced collaboration and coordination between key stakeholders to improve health outcomes.
Additional Comments In addition to routine tasks, response to emergency situations may be required and at times require after-hour communication with you. Your onsite presence may at times be required. Travel, including travel to areas that may not be served by public transportation is required.
Permanent, competitive, full-time position. Transfer applicants may be asked for a Social Security number to verify transfer eligibility. Travel throughout New York State, including to areas not served by public transportation, is required (~30%). Currently, Civil Service has approved for this title an increased hiring rate equivalent to the step four of the PS&T G24 salary schedule, and an additional $20,000 geographic pay differential.
