The OPAT nurse work in partnership with group of Infectious Diseases physicians, home care agencies, pharmacists, medical assistants and administrative staff to provide compassionate and comprehensive care for our vulnerable population.
Nurses utilize the nursing process in the delivery of care to prioritize patient needs. Services include but are not limited to care coordination and navigation services, triage of patient’s phone calls, liaison between patient and provider, discharge follow-up calls and assessment, patient education and participation in program-based activities. The OPAT RN performs an important care coordination role in the critical transition of care from the hospitalized inpatient to ambulatory outpatient follow up for patients in conjunction with the home care agencies. As part of OPAT, management of active lists of patients who are on parental antimicrobial therapy, lab tracking, etc. Program activities also include communication with providers at all levels of care, participation in multidisciplinary rounds.
Job Responsibility
- Provides health education; stratify and Manage risk
- Coordinate care with internal and external providers
- Actively participate with a multidisciplinary team
- Assists with patient education
- Applies clinical expertise and medical appropriateness criteria to provide high level overviews of patient outpatient antibiotic therapy plan, recent labs, diagnosis, and more, while working in conjunction with providers and home care agencies and Interventional Radiology.
Job Qualification
- Graduate from an accredited School of Nursing. Bachelor’s Degree in Nursing, preferred. Must be enrolled in an accredited BSN program within two (2) years and obtain a BSN Degree within five (5) years of job entry date.
- Current License to practice as a Registered Professional Nurse in New York State required, plus specialized certifications as needed.
