POSITION SUMMARY:
Save Our Communities (SOC) draws on evidence-based community violence intervention and prevention (CVIP) strategies that aim to reduce and prevent shootings and other serious community violence through the use of public health strategies. Focused on communities, individuals, and networks experiencing or engaging in violence, the Outreach Specialist works to address violence by:

  1. Coordinating and liaising with school staff, SOC School-Based Interrupters, and other appropriate partners to identify and provide coordinated support and mentorship to youth at the highest risk of violence victimization or perpetration.
  2. Working closely with a team of school staff and local partners to respond to incidents of violence that damage school-community norms, student body sense of safety, and may otherwise result in violent retaliation.
  3. Working closely with a broad coalition of local partners to promote school safety, positive school-community norms and increase school-community collective efficacy.

The Outreach Specialist is responsible for case management, system navigation, and advocacy for students, families, and networks of youth identified as high risk for violence victimization or perpetration. The Outreach Specialist also provides supervision of the interrupters and assistance to the Commissioner as needed. This is a grant-funded position.

ESSENTIAL JOB FUNCTIONS:

  • Promote proactive responses to prevention of violence with youth at high-risk of violence victimization or perpetration.
  • Case management, service navigation, and wraparound support for youth at high-risk of violence victimization or perpetration.
  • Assist Interrupters in maintaining active caseloads.
  • Collaborates in the development of transition plans for successful re-entry of suspended or justice system involved youth back into school-community.
  • Identify community resources and support services.
  • Identify and create partnerships with service providers.
  • Inform Interrupters of any relevant community activities
  • Monitor cases by verifying participants engagement; advocating for needs, services and entitlements.
  • Maintain participants records by recording case notes, events and progress in data management system.
  • Attend all staff meetings and other internal/external meetings as directed.
  • Participate in relevant training programs and seek out other tools for effective and efficient work productivity.
  • Perform other duties as assigned.

REQUIRED KNOWLEDGE, SKILLS AND ABILITIES:
Knowledge of:

  • Intimate knowledge of the earmark area (It is preferred that the applicant be from the community they are applying for.)

REQUIRED EDUCATION, EXPERIENCE, AND CERTIFICATIONS:

  • Bachelor’s degree and/or equivalent experience (4 years of related work experience), in a Human Behavioral Science such as Social Work, Psychology, or closely related areas.
  • Three (3) years successful experience providing case management for youth, including developing, monitoring, and updating individual case plans.
  • Experience working with children exposed to trauma
  • Ability to work a flexible schedule, including late evenings, weekends, and holidays.
  • Committed to working toward positive community change.

PREFERRED EDUCATION, EXPERIENCE AND CERTIFICATIONS:

  • Reside within the target area preferred.
  • Experience, training, or certifications in case management, as a Community Health Worker, in Trauma Informed Care, resiliency, Positive Youth Development, Restorative Practices, or other relevant youth and mental health certifications.
  • Experience working with high-risk youth and gang members.
  • Commitment to work from a strength based, trauma informed youth and family development perspective

The City of Toledo is a self-funded medical plan provider that offers a broad network of health services options for its employees. The City also offers a multi-tiered prescription drug plan and dental and vision care services.

Employees are required to pay a monthly premium depending on the employee’s selection of single, single plus one, or family coverage. Ohio Public Employees Retirement Plan

The City is a member of the Ohio Public Employees Retirement Plan (OPERS). All employees who are paid in whole or in part by the state of Ohio, a county, municipality, or any other political subdivision of state or local government in Ohio must become members of OPERS unless they are covered by another state retirement system in Ohio.

OPERS is a pension plan providing fixed retirement benefits every month based on a formula that rewards years of service. The City contributes 14.00% of the employee’s annual base salary and employees contribute 10.00% of their annual base salary.

Deferred Compensation & 401(K)

The City of Toledo offers a voluntary governmental 457(b) deferred compensation plan, which is a retirement savings plan that allows eligible employees to supplement any existing retirement/pension benefits by saving and investing pre-tax dollars through salary deferral.

Contributions and any earnings are tax-deferred (both federal and state income taxes) until money is withdrawn. Withdrawals are taxed at ordinary income levels.
