Why does the job exist?
The Social & Community Service Coordinator – Advanced will:
- Assist in managing allocations for the Centennial Care Community Benefit (CB) Program;
- Be responsible for assisting in approving exception allocations and developing and extracting reports from the MAD Tracker system for management of the program;
- Collaborate with and serve as a technical consultant to the Aging and Long Term Services Department in addition to staff across the division;
- Conduct research to determine root causes of Individuals’ eligibility issues that require reviewing cllent records using the Medicaid Management Information System (MMIS) and the eligibility system ASPEN;
- Work with and consult with the Income Support Division and managed care organizations (MCOs) In order to resolve the issues;
- Develop, revise, implement and interpret Medicaid policy in addition to working with multiple systems that include MMIS, ASPEN, SAMS central registry system, and the MAD Tracker system;
- Maintain detailed spreadsheets that track allocated members as they complete the process in order to receive CB services.
How does it get done?
The Social & Community Service Coordinator-Advanced is responsible for:
- Managing community reintegration (CRI) allocations, this includes communication to nursing facilities to validate that individuals meet the program requirements to be eligible for a CRI allocation;
- Researching, addressing and resolving complex eligibility and enrollment Issues;
- Having knowledge of ASPEN and the MMIS and how they intersect with Medicaid policy and procedures;
- Collaborating with contractors, contract managers and other staff to ensure the accuracy of the changes or corrections with a final result of appropriate services and benefits to clients;
- Conducting testing in ASPEN regarding new enhancements;
- Creating and analyzing reports, developing and maintaining tracking spreadsheets;
- Making process and policy recommendations to bureau management, and serve as a technical advisor that includes developing training materials and conducting presentations for ALTSD;
- Working with the Medicaid Management Information System Replacement (MMISR) team to develop new allocations processes.

The Ideal candidate has:
- Strong customer service skills;
- Knowledge of the Medicaid program;
Minimum Qualification
Bachelor’s Degree in Social Work, Psychology, Guidance and Counseling, Education, Sociology, Criminal Justice, Criminology and/or Family Studies/Services and four (4) years of any combination of experience including working with communities, working on health or social service related matters, social work/case management experience, behavioral health and/or health care. Working Conditions
Work is performed in an office setting: late hours and weekends work may be required. Will be exposed to regular periods of video display terminal and keyboard usage and stressful situations. Travel may be required. Incumbent will work under stress and frequent time constraints. Must have own mode of transportation. Incumbent may be required to furnish a personal vehicle for carrying out assignments.
