SOCIAL REHAB SUPERVISOR – Shoreline
BHcare is currently seeking a Social Rehab Supervisor to provide opportunities for empowerment, socialization, enjoyment and community involvement. To oversee the day to day operations of the Valley Social Club, ensuring compliance with contract requirements
Accountable and responsible for the following results:
Direct Service:
- Supervise staff activity at Valley Club.
- Supervise Community Warm Lines.
- Plan, develop, and implement recreational and leisure activities based on consumer needs and interests.
- Serve as leader/ co-leader of psychosocial groups.
- Oversee psychosocial programming.
- Assess individual situations and needs, coordinate services with CSS, and share pertinent information with treatment team members.
- Maintain flexible schedule to meet consumer needs.
- Report any significant behavioral alterations or symptoms immediately to the appropriate team leader, or other appropriate personnel.
- Encourage and assist consumers in becoming active participants in the community.
- Maintain records in accordance with policies and procedures.
- Facilitate the tracking of statistical information needed by the agency and funding sources.
- Monitor Lunch Program
Quality Management:
- Adhere to the standards of care to insure that the identified quality measures are obtained
- Supply data in a timely fashion to the Clinical Team Leader(s) to fulfill reporting requirements
Other:
- Attends training sessions, workshops, seminars as requested by supervisor, or as needed for professional development, or as required for accreditation or licensing.
- Required to use own vehicle on company business or to transport clients
- Regular and consistent attendance is an essential function of this position
- May perform other assignments and duties as assigned

Qualifications
- Bachelors Degree and one years experience or Associates and three years experience working with Prolonged Mental Illness.
- Certified Recreational Therapist preferred
- Must be proficient with PCs and Microsoft Office Word & Outlook
- Must be able to obtain and maintain a Public Service License
- Must have and maintain a valid CT drivers license if relocated to CT as a state resident more than 30 days ago. Must maintain an acceptable driving record and be insurable at a reasonable rate under the Organizations auto insurance plan.

- CPR and First Aid Certification
