Responsibilities:
The Social Work Care Manager provides psychosocial support counseling and other social work related services to Palliative Care patients and their families in both community homecare and facility settings. The Care Manager serves as a member of the Palliative Care interdisciplinary team (IDT), providing comprehensive psychosocial report to the team. In an effort to provide optimal case management services to patients and families, the Social Work Care Manager develops the palliative social work plans of care, the goals associated with those plans and makes recommendations based on patient/family needs for an active caseload. The Care Manager refers patients to Hospice whenever appropriate. Please note that this position will require travel to all boroughs as needed.
Qualifications:
- Graduate from the an accredited School of social work
- New York State LCSW licensure requred
- Minimum of two years experience in Social Work in a healthcare setting (Palliative Care experienced strongly preferred)
- Knowledge of Word and excel preferred
- Valid NYS Drivers license required
- Ability to work within an interdisciplinary team environment
- Possess excellent organizational, communication and listening skills
- Ability to input clinical data proficiently
