Job Description
Assists Social Workers in the delivery of social work services to patients and/or their families. Performs assigned tasks to assist with quality care and patient throughout.
Job Responsibility
- Assists social worker in service to patients and their families utilizing social work modalities in order to help clients maximize their functioning.
- As directed by social work team, coordinates discharge and post discharge services (authorization, community services, medications, prescriptions, transportation, etc.).
- May use clinical discharge plan to help identify appropriate resources.
- Assists in referral process by transmitting required patient documentation using web-based system.
- Communicates with insurance companies to facilitate care and secure authorization, as required.
- Performs diversified administrative duties supporting the operations needs of the Social Work Department.
- Compiles and analyses information summarizing departmental activities and prepares statistical data for reporting purposed as requested by management. Functions in liaison with all pertinent individuals or agencies that have an impact on patients in the community.
- Maintains appropriate professional patient care records and writes reports/letters pertaining to patients.

Job Qualification
- High School Diploma or equivalent required.
- 1-3 years of relevant experience, required.

The salary range for this position is $32,800-$48,540/year
