The Cumberland County Department of Social Services (DSS) and the Cumberland County Department of Public Health (CCDPH) have received funding to implement an initiative which focuses on primary prevention by utilizing a whole person/family approach and addressing social determinants of health (SDOH). The initiative includes multi-disciplinary Case Management and Care Coordination teams responsive to community referrals to provide care coordination and case management services to address social determinants of health, address social needs, improve health behaviors and health outcomes, improve health equity, and reduce health disparities, reduce preventable hospital stays, and prevent and reduce child abuse and neglect.

This position will supervise social worker/case managers that work with children and families who have been referred to the prevention program. The supervisor in this position will be responsible for assisting in the development of program policies and procedures, insuring that social workers/case managers are adhering to program policies and guidelines. Providing guidance to social workers/case managers in their work with children and families. Establishing and implementing documentation and data metrics to determine program outcomes. Developing and maintaining partnerships with service providers in the community. Promoting the prevention program as needed at community events.

Examples of Duties
Duties for this position include, but are not limited to:

  • Designating new program referrals to staff
  • Guiding social workers/case managers on the development of needs assessment for families/children,
  • Meeting regularly with social workers/case managers to ensure program requirements are being met, including a review of documentation of work with program participants.
  • Participation in meetings with public health staff and project managers
  • Conducting meetings with program participants to address questions or concerns regarding providing services
  • Developing tools, documents, and materials for staff to utilize in their work with children and families

Employee is subject to be called upon anytime to help the agency respond to a public health emergency which may include, but is not limited to, outbreak investigations, staffing mass dispensing or immunization clinics, providing disaster relief, or other incidents (man-made or natural and intentional or non-intentional) as they arise, or any public health response requested by the Health Director.
Minimum Qualifications

  • Master’s degree in social work from an appropriately accredited institution and one year of directly related experience; or
  • Bachelor’s degree in social work from an appropriately accredited institution and two years of directly related experience; or
  • Master’s degree in a human services field from an appropriately accredited institution and two years of directly related experience; or
  • Bachelor’s degree in a human services field from an appropriately accredited institution and three years of directly related experience; or
  • Bachelor’s degree from an appropriately accredited institution and four years of directly related experience; or
  • an equivalent combination of education and experience.

Degrees must be received from appropriately accredited universities.

Knowledge, Skills and Abilities

  • Knowledge of methods and principles of casework supervision and training.
  • Considerable knowledge of social work principles, techniques and practices and their application to specific casework and community problems.
  • Considerable knowledge of behavioral and socioeconomic problems and their treatment and governmental and private organizations and community resources.
  • Considerable knowledge of the laws, regulations and policies which govern social work programs.
  • Ability to supervise, train, or orient lower-level social workers, students, interns, or other staff.
  • Ability to express ideas clearly and concisely and to plan and execute work effectively.

Must have or obtain and maintain a valid North Carolina driver’s license with an acceptable driving record. Background check and negative drug test are required.
