General Statement
The Social Worker, under general supervision, provides professional social work services to clients in a variety of settings including local human service agencies, correctional facilities, hospitals, schools, medical facilities, patient/client homes, etc. Work involves evaluating the client’s situation and his/her ability to deal with it, developing a social history, psychosocial assessment, service plan and/or treatment plan and follow-up. Services may include placement in a residential facility; basic counseling, support, and reinforcement in areas such as death and dying, employment, vocational, medical, health, or behavioral issues. Employee addresses such problems as adjustment to illness or disability, placement into an institution, rest home or nursing facility; financial concerns; socialization issues of the socially handicapped or those debilitated by age or illness. They may provide information to and confer with schools, natural parents and foster parents on rehabilitation plans, behavior management, and other problem areas or needs.
Employee may be involved in recruitment, evaluation, and training of foster parents and other care providers, provides care management services and coordinate with a multidisciplinary team, or provides care management services to pregnant Medicaid recipients; and may develop community or service resources and/or develop volunteer programs. In certain treatment settings worker participates on rehabilitation/treatment teams and provides the social work perspective. Positions at this level combine difficult services performed under supervisory guidance with relatively conventional services performed under more relaxed supervision.
Public Health positions are responsible for preparedness activities in the event of a natural disaster, disease outbreaks, and other threats to the health of the public. Individuals in this role will be required to participate in preparedness response activities which may include mass vaccination clinics and sheltering.
Essential Functions
POSITION SPECIFIC FUNCTIONS:
Adult Protective Services
Performs assessments to identify needs and establish an appropriate service/treatment plan.
Monitors phones and takes Adult Protective Reports, In-Home Support, Adult Day Care & Day Health and Adult Services Referrals, etc.
Provides supportive/ directive counseling to clients and families in addressing needs and in some settings behavioral approaches are used to maximize client involvement.
Serves as inter/multi-disciplinary team member.
Assess adults for assistance for In Home Services, Adult Day Care & Day Health, Adult Placement and Adult Day Health.
Refers clients and families to a variety of services to meet financial, psychological, emergency care, emotional, or other needs.
Assumes a case manager role to follow the client through agency and external services.
Monitors Adult Services caseloads and completes quarterly and yearly Assessments.
Completes full assessment on customers applying for the Work First program, evaluating their situation, gathering information on their education, certifications, work history, criminal history, mental health and/or medical history, domestic violence, skills, transportation, goals, desires.
Guides and directs clients towards finding employment by placing them in job search, vocational training courses, job readiness classes, volunteer positions, etc.
Maintains frequent contact with local college personnel, area businesses.
Provides transportation to/from interviews, placement locations, and doctor’s appointments.
Conducts home visits and completes follow-up appointments at least every 12 weeks.
Provides resource information for various needs and takes customers shopping for interview and employment attire.
Submits receipts and payment requests daily, weekly, and monthly.
Counsels, advises, encourages, and guides customers through their current situation to becoming employed and/or off agency programs.
Forms partnerships and works closely with outside resources including Goodwill, Cardinal Innovations, NCWorks, SPCC, Health Department, and Daymark.
Schedules Work First Orientation appointments and leads Work First Orientation weekly.
Attends case staffing at Turning Point weekly.
Facilitates and leads hardship extension hearings for Work First.
Monitors and documents Adult Care Homes (ACH) compliance with licensure standards.
Investigates complaints in ACH about general conditions or violations of standards. Reports violations of Residents’ Bill of Rights with the involvement of the N. C. Division of Health Service Regulation (NC DHSR).
Reviews and assures completion of licensure applications materials for submitting to the (NC DHSR).
Provides consultation and technical assistance toward upgrading care and services in ACH.
Assists in recertification of Adult Day Care/Health (ADC) and assistance of inspections for renewal of license.
Identifies any violations or complaints concerning the functioning of an ADC or ACH.
Provides assistance to caretakers and facilities that have the responsibility of entering older adults into ADC and ACH.
Assists NC Division of Health Services Regulations (NC DHSR) with an annual survey of each facility with the permission of the DSS Supervisor of DSS Program Manager.
Assures completion of civil rights compliance material and submits to DSS.
Provides consultation and technical assistance to ACH administrators concerning civil rights compliance and investigates complaints about civil rights discrimination.
Investigates unlicensed facilities which house aged and disabled persons to determine if subject to licensure with NC DHSR.
Supports the development and operation of the ADC Services.
Trains, licenses and oversees foster parents and foster homes for the agency.
Organizes various trainings for foster parents throughout the year to help them meet their training requirement for licensure and keep them updated on the most current foster parent information.
Works with families on a one on one basis to assess them, their families and their homes for appropriateness for fostering, including home visits, one on one and family assessments, review and completion of paperwork and guiding the family through the licensing process.
Documents direct client interactions, services development, and referrals in accordance with agency requirements.
Utilizes agency guidelines, specific federal or state standards, and principles of a variety of supportive counseling techniques to provide services to clients.
Performs other related duties as assigned.
Qualifications
MINIMUM QUALIFICATIONS
Education and Experience:
Adult Protective Services, Child Welfare, Crisis Assessment and Guardianship
Bachelor’s Degree from an accredited university and two (2) years of experience in social work; or equivalent combination of education and experience.
Licenses or Certifications:
Must have a valid Driver’s License and be able to maintain a safe driving history as defined by the Union County Vehicle Use Policy.

- Child Welfare Social Workers need NC Pre-service Training within 3 months of hire.

PHYSICAL DEMANDS
Must be physically able to operate a variety of machinery and equipment including computers, copiers, telephones, etc. Must be able to exert up to 30 pounds of force occasionally, and/or a negligible amount of force constantly to move objects. Physical demand requirements are in excess of those for Sedentary Work. Light Work usually requires walking or standing to a significant degree. However, if the use of arm and/or leg controls requires exertion of forces greater than that for Sedentary Work and the worker sits most of the time, the job is rated for Light Work.

LEAVE PROGRAMS
VACATION LEAVE
Vacation leave is accrued based on creditable years of service in the Local Government Employees Retirement System.
SICK LEAVE
For full-time County employees, sick leave accrues at eight (8.0) hours per month. Benefitted part-time employees accrue prorated sick leave based on a percentage of the full-time rate. Your unused sick leave balance can be applied as creditable service toward a service retirement under the guidelines of the NCLGERS.
SICK LEAVE TRANSFER FROM OTHER AGENCIES
Unused sick leave earned from another North Carolina Governmental Agency and/or entity, will be accepted and transferred to the County as follows: twelve (12) sick leave days will be transferred and added to the employees active sick leave balance after completion of six (6) months of employment. The remaining balance of sick leave documented for transfer at the time of employment will be recorded in the employees permanent personnel file. On the fifth anniversary of employment with the County, the remaining balance of transferred sick leave will be added to the employees active sick leave balance.

CIVIL LEAVE
Paid leave is provided to full-time or regular part-time County employees called for jury duty or as a witness of the court for the federal or state government, or a subdivision thereof.
FAMILY & MEDICAL LEAVE ACT
Federal law provides job protection and continuation of existing paid health insurance during 12 weeks of leave, paid or unpaid, to an eligible employee for qualifying events. These events can include birth or adoption, and serious illness of an immediate family member, or of the employee.
PAID PARENTAL LEAVE
Parental Leave provides the employee paid leave for the first eight weeks of approved FML for the birth of a son or daughter, or placement with the employee of a son or daughter for adoption or foster care.
ADMINISTRATIVE LEAVE
Educational Leave is available to regular full-time and benefitted part-time employees when participating in a job-related, short course, such as an update, not to exceed ten (10) consecutive working days.
VOLUNTEER TIME OFF
Volunteer Time Off will provide employes with 16 hours of paid time per calender year to volunteer at approved Union County community partner agencies.
