Some of the many programs and services offered are career counseling, skills training, education and literacy programs, employment services, and more.
Position Overview
MERS Goodwill’s supported living program is adding another case manager to the team! Our case managers in supported living work with individuals with intellectual and developmental disabilities with a focus on developing independent living skills such as budgeting, maintaining medical care, comparison shopping, etc. All services are documented in progress notes along with the service plan created with each participant, to work toward reaching their individual goals.
Essential Duties:
- Complete Independent Living Assessment and develop individual program plan based on the results
- Maintain a sufficient caseload of individuals requiring Supported Living services such that fiscal and outcome goals are met
- Assist consumers to develop individual service plan
- Provide services in accordance with individual plan, including support with budgeting, maintaining medical care, participating in community activities, etc.
- Maintain knowledge of services available to persons with disabilities
- Communicate as needed with funding source/referring counselor regarding services and progress towards goals
- Maintain written program requirements, including progress notes to be completed in a timely manner
- As needed support consumers in accessing community resources to meet client needs
- Maintain file as required by funding source and to meet CARF standards (e.g., client assessment and voucher)
- Maintain minimum average of 80% billed each week
- Other duties as assigned
Qualifications
Requirements:
- High school diploma required, bachelor’s degree in social work or related field strongly preferred
- Two plus years’ experience supporting individuals with disabilities required
- Requires ability to observe and work well with people, and have excellent written/verbal communication skills
- Comfortable with working in community 70% of the time and traveling across the St. Louis area
- Current driver’s license and insurance required
- First aid/CPR and medication certification required, or will be provided at the start of employment
