Description

RESPONSIBILITIES:

Under the supervision of the Director & Assistant Directors of Clinical Services, the Transitional Age Youth Residential Linkage & Triage Coordinator [TAY Coordinator] will identify & assess clients in the community for eligibility/medical necessity for residential treatment. They will be responsible for assessing client mental health status, collecting collateral information from outpatient providers and triage to the appropriate Progress Foundation residential setting. The TAY Coordinator will work in partnership with and provide training to the youth service providers in San Francisco to support the readiness of the client to access residential treatment. The TAY Coordinator will provide clinical case management services for the clients referred through them and placed in ADU programs.

The TAY Coordinator is also responsible for the following duties:

  • Must have the ability to assess and diagnosis when needed, as well as initiate, review, develop and document psychosocial assessments of individual referrals.
  • Develop and maintain relations with other organizations providing TAY services, with the intent of supporting TAY clients experiencing homelessness in the community and providing diversions from County Jail and inpatient settings into Progress Foundation programs.
  • Work in concert with the Behavioral Health Service TAY Linkage collaborative providing consultation and linkage assistance.
  • Function as a community point of contact and reference for providers referring TAY for Residential Treatment Services
  • Maintain charting and other essential documents related to referrals and referral sources. Collect, analyze and make recommendations based on data trends.
  • Ensure that Progress Foundation programs get timely, adequate and accurate information to facilitate the referral and integration of clients into the program.
  • Upon assignment, participate in Progress Foundation sponsored or approved training opportunities, either as a presenter or as a participant.
  • Promote the social rehabilitation philosophy among program staff and clients.
  • Develop and maintain good working relationships with all client referral sources and other related educational, vocational and mental health services.
  • Educate non BHS TAY providers about Residential Treatment Services
  • Attend staff meetings, supervision sessions and other programmatic and/or administrative meetings as assigned by the Director of Clinical Services and Assistant Directors of Clinical Services.
  • Serve as a role model for positive employee relations, professional functioning and for adherence to the principles of social rehabilitation.
  • Comply with all applicable federal, state and local governmental laws and regulations, as well as all Progress Foundation policies and procedures.
  • Comply with the Progress Foundation Code of Ethics.
  • Inform the Director of Clinical Services and Assistant Director of Clinical Services of any personal events and/or circumstances which may affect the ability to perform the proscribed job responsibilities and functions or to meet minimum qualifications.
  • Perform other related duties as assigned by the Directors of Clinical Services and Assistant Directors of Clinical Services.

Qualifications

MINIMUM QUALIFICATIONS:

  • Master’s degree in Social Work or Psychology and registered intern with the BBSE (license eligible).
  • Specific experience in adolescent/young adult and/or knowledge of social rehabilitation is preferred.
  • At least two years’ experience in a community mental health program is required.
  • General case management skills, plus good clinical judgment specific to treatment planning, crisis assessment/intervention and milieu therapy are expected.
  • Must be able to communicate effectively with staff, clients, and other providers both orally and in writing.
  • Must demonstrate good judgment, and emotional stability.
  • Must be able to negotiate several flights of stairs during each work shift.
  • Must be computer literate and be prepared to enter clinical information in paperless charting software. Avatar experience a plus.
  • Must be free of conviction (and remain so) for offenses deemed, in the sole judgment of management, to adversely affect capacity to provide safe and competent services to program clients. Must clear (and maintain) fingerprint clearance.
  • Must maintain a valid California driver’s license and must be insurable under the agency’s prevailing auto insurance policy.
  • Must remain eligible for staff-level access to SFGH and In-patient services, as well as entry into the County jail.

Upon receipt of a job offer and before commencement of employment, must submit to a fingerprint check and health screening exam in order to obtain clearance from the State Dept. of Social Services, Community Care Licensing. Must have a valid California Driver’s License, and must be insurable under the agency’s current auto insurance policies. Must be able to provide own car.
