The Sustain and Grow Lead is a critical member of our ministry that will serve by providing case management for women to assist them with their individual needs to achieve secure housing. This is done by helping the client meet their benchmark goals through the development of a communal environment and a healthy level of accountability and structure.

Essential Duties and Responsibilities:

  • Conduct assessments with clients to determine goals, barriers, resource needs, and other factors related to case planning, acting as a broker for services and referring them to community resources when necessary
  • Creates independent living community that is conducive to clients remaining rooted in the community and relationally connected with others
  • Regularly goes into the community to build new relationships with community partners in order to secure additional resources for clients’ needs
  • Facilitates internal community events that support independent living, social, financial, and recovery skills
  • Conduct orientation to help acclimate new clients to Sustain & Grow protocols
  • Research and remain current in knowledge of critical resources such as medical, legal, childcare services, and others in the Atlanta area
  • Inputs, tracks, and reports client outcome progression and client engagement data
  • Review and evaluate caseload monthly to track client progression toward outcomes
  • Coaches and ensures recommended services are personalized to achieve outcomes
  • Leverages motivational interviewing to engage with clients to determine needs for external referrals
  • Serves as a liaison for clients connecting with their Make Progress clinical team for life adjustment support

Knowledge, Skills and Abilities:

  • Experience connecting clients to resources
  • Knowledge of medical, legal, and childcare services in the Atlanta area
  • Crisis intervention experience
  • Proven ability to be a self-starter
  • Ability to work cross-functionally and collaboratively
  • Experience in motivational interviewing

Physical Requirements:

  • Must have the ability to remain in a stationary position (standing, walking and sitting)
  • Must be able to communicate accurate information
  • Must be able to occasionally lift and/or carry up to 10lbs
