Provide program oversight and coordination of transition focused services for older adults and individuals with disabilities seeking transition from nursing facilities to the community. This includes provision of options counseling for all referrals to ensure comprehensive discussion of individual’s unique situation, resources and goals. Transition services includes determination of eligibility, Social Determinants of Health (SDoH) screening, Social Isolation screening, and coordination of referral and access to services available. Provide referrals to service providers such as community service providers, Home and Community Based Services (HCBS) staff, Assistive Technology of Kansas (ATK), Independent Living Resource Center (ILRC), housing providers and others as needed. Responsible for tracking client data, expenditures, service provision and outcomes. Ensure that budget is tracked and services are provided within the program guidelines and funding allocated.
Service Provision
- Direct services: Options counseling, conduct SDOH and Social Isolation screening, determine program eligibility, assist with locating and obtaining necessary resources, and make referrals to service providers as appropriate.
- Coordination: Coordinate with all service providers, department staff, nursing facility staff, housing providers, KDADS and MCO Transition Teams to ensure seamless transitions.
- Provide consultation to all service providers and maintain clear and consistent communication regarding client issues and needs.
- Marketing: Work closely with the Client Services Director to develop marketing plan, messaging, and implementation plan to ensure effective marketing of the Transition Program and services.
- Provide outreach and education to older adults, caregivers, nursing facility staff, and partners in the tri-county area (Sedgwick, Harvey and Butler Counties) to increase program referrals and knowledge of the service.
- Maintain documentation of all services provided, case logs, purchases, billings and tracking of progress and outcomes.
- Complete required case documentation, billing and reports on a timely basis.
Specialized Program Coordination
- Coordinate with and assist the Client Services Director in the development and implementation of the program with the goal to enhance delivery of services to older adults, people with disabilities and caregivers.
- Provide assessment of referral procedures, timeliness of service provision, access and successful utilization of technology, assessment of housing resources, and transition assistance, referrals to other agencies or internal programs.
- Communication with organizations, program managers and financial staff.
- Participation in team meetings.
- Community education and outreach to older adults, caregivers and professionals to promote the transition program and the ADRC Call Center.
Marketing and Messaging
- Assist with continual refinement of a marketing plan for the Transition program.
- Coordinate with Client Services Director to identify venues for marketing the program.
- Monitor the referrals and reach into the community
Minimum Qualifications: Bachelor’s Degree in Social work, gerontology, psychology or related field. 2 years of experience in social services field focused on working with older adults and individuals with physical disabilities. Per Sedgwick County policy, this is a driving level position that requires a valid US driver’s license without restrictions and current proof of automobile insurance.
Preferred Qualifications: Master’s Degree in Social work, gerontology, psychology or related field. 2 years of experience in program management. Licensed Social Worker. Bilingual in English and Spanish.
