VACCINE EQUITY NURSE HEALTH NAVIGATOR – BISBEE/COUNTYWIDE

RATE OF PAY: $25.00 / hour, plus a comprehensive benefits package to include membership in the Arizona State Retirement System

PLEASE NOTE: Position is grant funded with employment contingent upon continued grant funding.
Under general supervision, performs specialized support activities in support of the Vaccine Equity Grant in collaboration with the Vaccine Equity Program Coordinator; administers vaccines; provides patient education and assists community members in navigating available resources; performs related work as assigned.
Essential Functions
Assists the Program Coordinator with program specific surveys and data collection to formulate and achieve objectives, goals and interventions within the Vaccine Equity mission and vision; Establishes and maintains relationships with stakeholders within our county communities; Identifies offerings within the communities and gaps in needs; Represents the Vaccine Equity program for the county as well as outside agencies and vaccination providers; Promotes all services and programs offered by Cochise Health and Social Services; Creates and distributes educational materials; Provides ongoing evaluation of services provided to citizens of our communities to include quality improvement suggestions to enhance the Vaccine Equity Program.
Minimum Qualifications
Must possess and maintain an unrestricted license in good standingas a Licensed Practical Nurse (LPN), issued by the Arizona State Board of Nursing; one (1) year of experience performing direct patient care and/or community health nursing as an LPN or RN in a clinical, public health, home health or hospice nursing position; must possess current CPR certification and Basic Life Support card or obtain prior to beginning employment; must possess and maintain a valid Arizona driver’s license and the ability to be covered by county insurance. An equivalent combination of education and/or experience from which comparable knowledge, skills and abilities have been achieved may be considered.
Knowledge, Skills & Abilities
Considerable knowledge of:
- principles and practices of nursing;
- principles and practices of public and community relations;
- public health issues, concerns and needs;
- communicable disease programs and public health programs.
Good knowledge of:
- community medical and social services resources and their availability;
- modern office practices and procedures.
Working skill in:
- Vaccine administration;
- documentation in electronic health record system;
- Microsoft Office;
- preparing and delivering medical/public health training and information directly to patients/clients, their families and to the general community;
- maintaining appropriate confidentiality of information encountered during performance of duties.
Ability to:
- establish and maintain effective working relationships with co-workers, other agencies, and the public;
- follow written and verbal instruction;
- educate and demonstrate Vaccine Equity activities;
- undertake a variety of both administrative and medical tasks;
- read, write and speak English;
- communicate effectively verbally and in writing;
- drive/travel to various work sites within the county.
WORK ENVIRONMENT: Work is primarily performed under general supervision. Incumbent generally performs work in a typical clinical or office setting and includes possible exposure to needles and blood borne pathogens. Work may be performed outdoors in a variety of climatic conditions. Weekend and evening work may be required. Travel required.
PHYSICAL REQUIREMENTS:
Essential functions of this position require: lifting, manual dexterity, large motor skills, ability to communicate.
Medium work: Exerting up to 50 pounds of force occasionally and/or up to 20 pounds of force frequently or constantly to lift, carry, push, pull or otherwise move objects, including the human body; involves sitting only occasionally, walking and standing are required regularly, incumbents may be required to kneel, crouch/squat, crawl, climb, stoop, turn/twist, balance, reach, or handle.
Mental Application: Utilizes memory for details, verbal instructions, emotional stability, critical thinking, adaptability and creative problem-solving skills are important.
