What will you be doing in your role?
- You advocate for clients with local service providers and within the community.
- You work collaboratively with team members and support the overall goals of the NW Services team.
- Your knowledge of community resources allows for strong, successful referrals for clients.
- Develop individualized service plans with each client, helping them see a path toward a future of independence.
- You will ensure professional service delivery by providing timely case management, documentation, and communication with clients.
- You will regularly meet with the clients to discuss progress towards their goals and celebrate their successes.
- You will engage clients and help them understand their value and dignity.
- You will walk alongside the clients on their path to success while cheering them on and offering guidance.
Are you the next Veteran Services Case Manager?
- Bachelor’s degree in social work or related field, preferred.
- 3 years of Case Management experience, preferred.
- Experience working with people experiencing homelessness, preferred.
- Must have reliable transportation, a current Texas Driver’s license and evidence of auto liability insurance.
